The Cardiovascular Management Program for Heart Disease is available to all our members 18 and older who have been diagnosed with heart disease. If you have been diagnosed with heart disease, you will be automatically enrolled.
This program will help you understand heart disease, how it affects your life and how you can better manage your condition. A few times each year, you’ll receive our Lifestyles for Heart Disease newsletter that will help you:

Case managers provide individual education and counseling, and coordinate care with physicians, for members who need complex care.
