- The lifestyle score, which summarizes the impact of daily behaviors on your health
- A tailored action plan to help you meet health goals
- Recommended digital health coaching programs that support you while you make lifestyle changes
Accessing the health assessment is easy. Here's how to get started.
- Log in to Member Secured Services.
- Click on Health and Wellness, and then on My Health Assessment.
- Click Start Now to begin the questionnaire.
